Hospice massage is provided solely for comfort, relaxation, and supportive care. Each session is individualized and adapted to the person's condition, comfort, energy level, and preferences.
The purpose of hospice massage is not to diagnose, treat, or cure any medical condition and is not a substitute for medical care, nursing care, or hospice services. Techniques may be modified, shortened, or discontinued at any time to prioritize the patient's comfort, safety, and well-being.
Participation is voluntary, and consent may be withdrawn at any time.
I have read and understand the information above. I understand the nature and purpose of hospice massage and voluntarily consent to receive these services. I understand that no guarantees have been made regarding specific outcomes or symptom relief.
If I am signing on behalf of the patient as their legally authorized representative or Medical Power of Attorney (MPOA), I certify that I have the authority to provide consent for these services.
For everyone's safety, please secure household pets in another room during the session unless they are a trained service animal.
